Reminders regarding 1095Cs and 1095Bs issued by UC/UnifyHR and UC’s insured health plans

TO ALL EMPLOYEES:

As a reminder, the 1095C form is not required for filing a tax return.  Likewise, for those employees who’ve already filed their return, receipt of the 1095C will not necessitate an amended return.

Reminders regarding 1095Cs and 1095Bs issued by UC/UnifyHR and UC’s insured health plans

  • Only ACA-full time employees (those who work 75% time or more) will receive the 1095C from UC Parts I and II of the 1095C will be populated for all employees; only those enrolled in UC’s self-insured plans (Core and UC Care) will have Part III populated.
  • All other plan members of UC’s self-insured plans (Core and UC Care) will receive Form 1095-B
  • All plan members of UC’s insured plans (Health Net, Kaiser, WHA, UC Health Savings Plan) will receive Form 1095-B directly from the plan.
  • All employees from PPS locations will be able to access electronic copies of 1095s on AYSO – even those who did not elect electronic delivery. These electronic copies will be available on or before January 31st, 2018 for those who need the information for their taxes.
  • UnifyHR will be mailing the paper copies of the forms to anyone who did not elect electronic delivery by January 31st, 2018.
